The FDA approved treatments for advanced Parkinson’s disease are:
(1) best medical therapy with medications
(2) infusion therapy with Apo-morphine
(3) Deep Brain Stimulation surgery
Gamma knife surgery is not part of the recommended PD treatments.

Dear Dr. Chee,
I’m a 38 years old male, non smoker non drinker.
About 2.5 months ago while brisk walking half way i suddenly experienced little dizzy and very rapid heartbeat and then both of my palm were numbed and sweating. I was admitted to hospital and shortly after arrived hospital my condition improved. Then i have done chest xray, EKG, stress test, holter monitor for 2 days and angiogram and all come back ok. Cardiologist think my heart has no problem. But since then i persistently feel little dizzy (non spinning type) or disequilibrium whenever i move/walk. If i just sit still i feel alright. Whenever the dizziness start or gets worse i always have this strange feeling of goosebump/cold/pulling at the back of my head and the back of my neck will feel tight too.
About a month ago while walking around shopping mall i suddenly experienced little dizzy and very rapid heartbeat and then both of my palm were numbed again. I was sent to hospital again and done ekg, blood test and found nothing wrong. Doctor suggest it should be panic attack. I have went to see ENT and he suspect it’s either BPPV or meniere’s disease and suggest to take Betahistine. I took it for 2 weeks but doesn’t seems to have any effect. For info, i also has tinnitus at my left ear (now both ears) started 2 years ago but has improved 70-80% now where i can only hear it in very quiet place.
Sine then my anxiety level is very high that even going out from my house makes me nervous and heartbeat fast. I took Pharmagaba for a week and now my anxiety has been much lower and i can go out without feeling nervous. But the dizziness still persists. Since i always have this strange feeling of goosebump/cold/pulling at the back of my head and tight neck i am thinking if the dizziness is of neurological cause?
I have actually visited you before when i’m about 20 years old due to a strange hot feeling of my backhead and sick feeling which persisted after seeing a few other doctors. And at that time you have ordered me to do MRI and found that there is infection at the middle of my sinus. You prescribed good antibiotic for me and has cured my problem.
Hope to have your opinion this time and considering to visit you again soon.

You are advised to consult with your GP to assess whether you should get repeat neuro investigations. Panic attack should be considered once the tests confirm there is no physical illness.

My Mother recently diagnosed of having bleeding in brain due to high blood pressure.and she had operation on last tuesday ( 04/08/2015 ) morning on her right hand side brain. Until today she is still unconcsious..we family members are eagerly waiting for her to be conscious.we are so worried because until now we have no idea or latest results of her current condition..we are almost day and night visiting her but really not sure exactly what is her current condition..and we too doesnt know to consult which doctor is incharged..i hope doctor

My dad is 66 years old. His MRI report as belowdecks: the spinalcanal is widened at the level of T12. It measures about 16x23mm. There is an extradural mass at L1/2. It measures about 1.6×1.3cm at it’s widest diameter and extends about 3.9cm. The mass has mixed intensities on T2 and isointense on T1 weighted images. There is intense enhancement of the mass after contrast administration. The spinal cord and the nerve roots are displaced to the right at this level. There are no other intraspinal enhancing lessions. The spinal cord ends at T12/L1. The exit foramen are not widened. There is normal thoracic vertebral alignment. The vertebral body contours and marrow signal intensities are normal. There is no loss of vertebral body height. The disc heights and disc intensities are relatively maintained. No disc herniations. There are no thickenings of the ligamentum flavum or face hypertrophy. There is no disruption of the anterior and posterior longitudinal ligaments. There are no paravertebral or prevertebral soft tissue masses.
IMP: features would be suggestive of an extradural mass at T12. Possibilities would include a meningioma or neurofibroma.
He has no issue to go the toilet, however, he walks slow and unstable now.
Please kindly advise what kind of treatment is suitable for him? Is spine operation is very risky? How much is the estimate cost?

Good day!
My father was admitted to hospital last June 15 and was then diagnosed with a growth in his right brain, doctors are not able to diagnose if it’s a benign or malignant growth and has suggested for surgery to remove the growth. However, the doctors did mention that it looks like a malignant tumor as per their experience.
We have tried to schedule for surgery for 4 times but was being postponed for all 4 due to no ICU room available in the hospital. It was very stressful and my father in the end decided not to go for the surgery.
We do not see improvement in his condition and we knew that there is a new technology – Gamma Knife, that can help his illness.
Is there any risk in doing gamma knife? My father is 62 year-old this year, and he is getting very depressed since the day he was discharged from hospital. His left leg is now swollen. His left body is weak. He was still able to walk very slowly, to walking with the aid of walking stick until now, he is on wheelchair most of the time. We are very worried about his condition but we do not know what can we do to help him.

Thursday he is suddenly no energy and can’t walk. I bring him to GHand go thru CT scan. I scare he will stroke. Base on the report, GH doctor said that is not stroke and just have a sease in brain. But today he back to no energy and tired all the time. Clinical doctor check his diabetes is 10. I keep on saw him tired want to sleep. What can I do ? Need send to emergency again ?

Your father’s doctor would be able to advise whether his tiredness is due to severe diabetes or Parkinson’s disease, or the result of both problems combined. Appropriate treatment will be based on this assessment.

Histopathology Report… Macroscopy – Fragments of haemorrhagic soft pale tissue 1cm in total; submitted all in 1 block. Microscopy – Section and levels show an infiltrating astrocytic tumour (GFAP-positive) exhibiting areas of moderate and marked hypercellularity. The tumour cells show moderate dysplasia, with presence of gemistocytes and pleomorphic spindle cells. Mitotic figures are noted (ki67 is between 10-30% in areas of increased cellularity). In the more cellular areas, there is also prominent micro vascular proliferation. Interpretation – Compatible with glioblastoma (WHO grade IV).

我儿子现在13个月大。5月头进行MRI证实是脑积水（hydrocephalus），5月尾进行了ETV手术， 7月2日伤口leaking，当时照了CT Scan，发现后脑的水少了，大脑里面的水还是一样，只是没对大脑造成压力（pressure)， 手术医生说目前还不需做shunt， 因为看起来有进展，但是我们还是觉得担心，因为他到现在还不会爬。另外通过CT Scan也证实是Dandy Walker Syndrom。手术至今2个多月，是否我们需要做MRI，以仔细看看真正需做shunt吗? 或多久过后才需做MRI？通常做ETV过后脑里的水多久才会明显看到减少与否？

I have neck slipped disc problem. I have been diagnosed by MRI results three months ago that I have multi levels disc disease. I have this problem since two years ago. I have constantly right shoulder pain because nerve compression from the herniated discs. My cervical disc from level C3 to C7 have been herniated and bulged from MRI reports. I have tried physio therapy but it failed to my case. I have got the consultation from the neurosurgeon and orthopeadic surgeon. Neurosurgeon advised me to do the artificial disc replacement surgery ( Anterior Cervical Discectomy ) for C4/C5 and C5/C6 . Orthopeadic surgeon advised me to do minimally invasive neck surgery ( Nucleoplasty). Neurosurgeon advised to me that my neck disc is phyte and firm, minimally invasive neck surgery cannot help me. In contrast, orthopeadic surgeon advised me that minimally invasive neck surgery can help me. At this moment, I need to make important decision to choose which surgery to go. May I seek your kind advice which surgery is suitable for my case? And what are the benefits and disadvantages between these two surgeries?

Central pontine myelinolysis is the loss of myelin sheath in nerve cells in the pons. Other areas of the brain can also be affected. To date, there is no specific medication. Treatment consists of supportive measures and there is possibility of improvement over time.

Top Urgent – seeking for helps and advice

I was having migrain since high school, more than 15 years. Now i am 32 years old, female.
1 week ago, I get minor strok / something similar to strok (but it is not strok after diagnose by doctor), after the serious migrain attack, and the heart and left hand feel numb and difficult breathing.
After CT Brain scan, found that a cluster of prominent vessels are seen in the right parieto occipital lobe with a nidus. Appearances suggestive of AVM. A rounded area of CSF density structure is seen in the right perietal lobe, either an old infarct (encephalomalacia) or extension of CSF space. No mass effect or midline shift. The ventricular systems, sulci and basal cisterns are not dilated. There is some prominence of the CSF space over the left frontal lobe. normal gray-white matter differentiation.
Later had done MRI, found the a cluster of abnormal vessels accumulated near the right eye brain and they may redirect too much pressure to 1 of the low pressure vessel at my right back brain and the vessel seems quite swollen inside the MRI photo. The actual MRI report still waiting to generate by Hospital Sungai Buloh.
Neuro Surgeon doctor in Sungai Buloh Hospital then suggested me to have soonest angiogram scan and treatment will depends the size and the condition of the AVM. The date angiogram is next Tuesday.
I wish to know how much percent for a patient with brain AVM to survive the angiogram scan? How many percent the vessels may break or bleeding during angiogram? If I used to involve in road accident 10 years ago and little bit hitting the heart at that time, will it make the scan process more risky and easily get strok or coma? I also experience 3 times surgery at the left foot sole to remove corn in Dec 2008, Jan 2009 and Oct 2010.
After the angiogram, what treatment may propose? How much cost and period of treatment?
How much successful rate in your hospital for the AVM patient to survive after the treatment? I mean the patient can live and work like normal people…
Who is the most experience with proven record neuro surgeon doctor in your hospital could recommend to us?
If I refuse to do angiogram scan and reject any surgery or embo or radiation treatment,
any conventional medical treatment can be offered to me, such as only taking medicine to prevent or do you have any kind of less risky treatment can be proposed?
